Dr. Erich Arias has been officially appointed as an authorized Civil Surgeon for the Department of Homeland Security by the U.S. Citizenship and Immigration Services (USCIS). He is qualified to perform green card or immigration medical examinations (I-693 Form).

This exam is straightforward and, in most cases, can be completed in a single office visit.

About the Exam

The following is a list of items we suggest that you bring to your medical examination.


WHAT YOU WILL NEED FOR YOUR IMMIGRATION EXAM

  1. Valid Government-Issued Photo ID
    Please bring a valid government-issued identification document, such as a driver’s license, passport, or any other form of ID you plan to use.

  2. Complete the Immigration Exam Registration Form
    You can complete our online "Immigration Exam Registration Form" ahead of your visit. For added convenience, we also offer a downloadable paper registration form if you prefer to fill it out manually.


OPTIONAL DOCUMMENTS

  1. Previous I-693 Forms (If Applicable)

    • If you have a previously completed USCIS I-693 Form, please bring it to your appointment.
    • You do not need to bring a blank I-693 Form.
  2. Your A-Number (If Available)

    • The A-Number is a unique identifier consisting of the letter "A" followed by eight or nine digits (e.g., A 012345678).
    • You can locate this number on any correspondence from USCIS.
    • While helpful, the A-Number is not required to complete your immigration medical exam.
  3. Proof of Prior Vaccinations (If Available)

    • Bring any records of prior vaccinations you may have.
    • If you don’t have proof, don’t worry—we’ll assist you in completing any required immunizations.
  4. Laboratory Test Results (If Applicable)

    • If you have lab test results related to a prior immigration exam, please bring them along.

NO PRE-VISIT LABORATORY TEST REQUIERED
  • You don’t need to complete any laboratory tests prior to your initial visit. As part of the USCIS medical examination, we will explain which vaccines and laboratory tests are required in a cost-effective manner.
