You may opt to register and enroll online by completing the forms below. This enables you to request and confirm an appointment with our clinic. Upon completion of your enrollment forms you will be asked to submit payment of $30.00 to secure your appointment time. This will be considered and applied towards your copayment, coinsurance, deductible and any other out of pocket expenses you may incur on your first visit with us. If you do not have out of pocket costs you may request a refund. 

Please be aware, no refund will be issued if you choose not to show for your confirmed appointment. Please take the time to enroll in your patient portal which will be available to you after the registration process is complete.
